Family Eldercare’s mission is to provide essential services to seniors, adults with disabilities and those who care for them. The $3,000 awarded to Family Eldercare will be used to pay a portion of the Case Manager’s salary and benefits in the Care Transitions Program. The Case Manager provides clinic and home-based assessments and care management for the clients of Lone Star Circle of Care who participate in the Care Transitions Program.
